well when i make requested figures i would do everything i would normally do, and then say at the end of the description that the figure was requested by *name*

but to answer your question…
1) i would love to mess around with them to see if i could make a cool thumbnail, but obviously it’s yours so u don’t need to use it

2) you made this. i did request it, but it’s not like i actually designed anything so no, credit not required
