    You repeatedly move your body so it gets tired and your muscles experience hypertrophy which is where your muscles tire out and begin to break down and re-construct themselves bigger, this is how exercise works.
    
    Sex would exercise said muscles due to hypertrophy and the feeling of it being good would override the pain to a high degree.
