Make his mouth hang open, make him go wall eyed but looking down, maybe add some distortion and I guess then you have it?

I think it’s Gorey enough but to make it more Gorey uncover the eyes and make them look torn open, or remove some of the hair to expose the skull or go even further and crack the skull open to show some brain. You could also try removing the jaw and make the flesh look torn
